Nurse Practitioner, Clinical Supervisor
As a Nurse Practitioner, Rayane serves as the primary care provider for a panel of about 75-100 patients and as the locus of care coordination for her panel on our interdisciplinary team. She visits patients regularly in their homes conducting new intake, follow-up, and annual wellness appointments and responds to requests for urgent care visits when new issues arise. Rayane prescribes medications, assesses care needs, writes care plans, orders DME and supplies and makes referrals to specialists when needed.
As a Clinical Supervisor, Rayane oversees the interdisciplinary team and our clinical processes. She ensures team members have what they need to do their jobs, provides coaching and training, cultivates effective team collaboration and instills the Feltin values in the way all of our team members approach complex care within the home setting.
Rayane has over ten years of experience working as a Nurse Practitioner in the community health center setting and in other home-based care practices such as Commonwealth Care Alliance and Landmark Health. Prior to that she worked at Spaulding Hospital as a nurse’s assistant and then as a registered nurse, and as a Teaching Assistant at MGH Institutes of Health. Rayane is a dedicated and compassionate provider who is focused on delivering high quality, holistic care utilizing evidence-based practice. She is passionate about expanding access to home-based care on a wider scale. Rayane earned her BA in Sociology from Boston College and her MS in Family Primary Care Nursing from the MGH Institute of Health Professions. Mrs. Salerno is certified as a Nurse Practitioner in the state of MA and as a Family Nurse Practitioner through the American Nurses Credentialing Center.
