As a Care Navigator, Raquel serves as the main point of contact for our patients, their families and other people on their care team. She schedules all visits, answers questions, triages clinical needs and routes to the appropriate team member as needed, ensuring timely response and excellent service. Raquel provides critical back office support to our practitioners in the field to keep things running smoothly and administers all of our claims submission and billing activities too.

Ms. Ozoria has over 10 years of administrative, customer service and billing experience in a variety of health care settings, including The Psychological Center, Home Health VNA and most recently with Commonwealth Care Alliance. Raquel is fluent in Spanish and is a certified medical interpreter. Raquel has been touched personally by the importance of our care model as she has a family member with complex needs.
