Colleen Thompson, MPAS, PA-C
Physician Assistant
As a Physician Assistant, Colleen serves as a community primary care provider and as the locus of care coordination for her panel on our interdisciplinary team. She visits patients regularly in their homes conducting new intake, follow-up, transitional care, and annual wellness appointments and responds to requests for urgent care visits when new or acute issues arise. Colleen assesses care needs, prescribes medications, writes care plans, orders DME and supplies and makes referrals to specialists when needed.
Colleen has been a Physician Assistant since 2012, caring for high risk and at-risk patients, both in the home-based care model founded by Dr. Master at CCA and as a hospitalist in hospital and skilled nursing facility (SNF) settings in the Merrimack Valley region. Colleen is kind, compassionate, and committed to patient-centered care. Ms. Thompson earned her BS in Health Sciences at Merrimack College and her MPAS at MCPHS University in Manchester NH. She is board certified by the NCCPA. Colleen is a Merrimack Valley native and enjoys spending time with her family.
